All persons with diabetes benefit from diabetes education as they learn self-management skills to control their diabetes and optimize their ability to live a productive and satisfying life. The diabetes educators at Hillcrest Hospital are committed to the concept of quality patient education. Our goal is to assist people with diabetes in gaining confidence as they embark on this lifelong journey of health care.
Our comprehensive diabetes management program, available to inpatients, outpatients and the community, is endorsed by the Ohio Department of Health as a "model program" and is a recognized education program of the American Diabetes Association.
The diabetes health care team consists of your doctor, certified diabetes educators, who are nurses and dietitians and yourself. The program consists of:
- Inpatient diabetes education services provided by certified diabetes educators
- Monthly, bi-weekly and bi-monthly diabetes group classes offered ( Review our upcoming courses)
- One-on-one teaching sessions with diabetes nurse educators covering diabetes management topics such as insulin instruction, blood glucose monitoring and problem solving
- One-on-one teaching sessions with registered dietitians certified in diabetes education, detailing meal planning, diet modification and weight control
- Free community diabetes education programs, offered four times per year, with guest speakers who discuss aspects of diabetes care and self-management.
Before your first diabetes education session, please print and fill-out these three forms. Don't forget to bring them to your first class!
