• Letters of recommendation
  Letters should be emailed by the letter writer directly to the Residency Program Director of the program, Mary Temple, and to the Pharmacy Human Resources Recruiter, Patty Onacila.
  On the application form, you will need to provide the name, relationship, email address and phone number of the three people that will be providing letters of recommendation.
• Curriculum vitae (CV)
  A PDF of your Curriculum Vitae (CV) should be emailed directly to the Residency Program Director of the program, Mary Temple, and to the Pharmacy Human Resources Recruiter, Patty Onacila. Please save the file you are uploading as a PDF document with your last name, first name, CV, and PGY1 as the file name.
