Biographical Sketch

Dr. Cook from graduated LECOM School of Pharmacy in 2006, a pharmacy practice residency in 2007, and has been a Board Certified Pharmacotherapy Specialist since 2008. He is currently licensed to practice pharmacy by the Pennsylvania and Ohio State Boards of Pharmacy. His current practice site is at Millcreek Community Hospital where he is a Clinical Pharmacist with an internal medicine service. Dr. Cook lectures in various courses at LECOM School of Pharmacy including Drug Information and Research Methods and Pharmacoepidemiology. Dr. Cook is Director of the LECOM / MCH PGY1 Pharmacy Residency Program, a member of the P&T Committee at MCH, a member of the admissions committee for LECOM School of Pharmacy, and a member of several professional pharmacy organizations both at the local and national level.

Biographical Sketch

Dr. Smith graduated from The Ohio State University College of Pharmacy in 2007, completed a hospital pharmacy practice residency, and has been a Board Certified Pharmacotherapy Specialist since 2008. She has worked as a clinical pharmacist with a focus in critical care since 2008 at South Pointe Hospital, a Cleveland Clinic hospital. Dr. Smith regularly educates physicians, nurses, and medical students, as well as precepts pharmacy student annual research poster presentations at the ASHP Midyear Meeting.

She was recently a recipient of the Cleveland Clinic Caregiver Celebrations Excellence Award in the 3rd quarter of 2010 for providing quality care to intensive care unit (ICU) patients. She has also developed and collaborated on several hospital and health system-wide policies and procedures. Dr. Smith holds faculty appointments at Ohio University College of Osteopathic Medicine, The Ohio State University, NEOUCOM and University of Toledo. She is also a member of several professional pharmacy organizations at local and national levels. Dr. Smith is on various committees including the Cleveland Clinic ICU Integration Team, Code Blue Committee, ICU Committee, Critical Care sub-panel of CCHS Medical Staff P & T Committee, Surgical Care Improvement Project Committee, and the Cleveland Clinic Antimicrobial Stewardship Working Group, amongst others. She also regularly supports pharmacy management needs within the hospital.

She has been a Board Certified Pharmacotherapy Specialist since 2010 and has been certified in Advance Cardiac Life Support since 2009. She is currently licensed to practice pharmacy by the Ohio State Board of Pharmacy. Her current practice site is at Hillcrest Hospital where she is the clinical pharmacy specialist in the medical and surgical intensive care units. Dr. Lagzdins has provided guest lectures at the University of Findlay on various critical care topics, and precepts pharmacy students from a variety of Ohio pharmacy colleges. Dr. Lagzdins is a member of the P&T Committee at Hillcrest Hospital and a member of several professional pharmacy organizations both at the local, state, and national level.
